Current Projects

ePRO Transitions

Patients with multiple chronic conditions are at increased risk for an adverse event (AEs) after discharge from the hospital.

This study uses electronic patient reported outcome (ePRO) measures collected via a mobile web app and data from the electronic health record (EHR) to give patients individualized risk scores and health advice to mitigate AEs during the first 30 days post-discharge.

CARESync

Our team is designing, developing, implementing, and evaluating methods empower patients to locate, collect, and share their EHR recrods for research.

This includes enhancing an existing application and digital infrastructure from the ePRO Transitions study to aggregate EHR data shared by older adults with multiple chronic conditions.
